Age Requirement: 0-6

Eligibility/Criteria: Respite support is offered to children up to the age of 6 who have severe neurodevelopmental delay.
Another key part of their service is end of life care which is provided for all children up to the age of 6 regardless of diagnosis.

Who can apply: They accept referrals from families, medical professionals, social workers, GP’s, Public Health Nurse’s and Early Intervention Teams.
Following receipt of a referral, the Liaison Nurse Manager in the area will contact the family as soon as possible.
